Responsibility Pledge

I understand that the Vivian Athletic Association is a volunteer organization and is run on a volunteer basis. With this in mind, I realize that it takes the efforts of every parent or guardian to keep the season going.

I understand that I am responsible for transportation for my child to and from all practices and games. Failure to have my child present at regular practices and/or games may result in my child being benched or suspended from play. I further understand that the registration fee is due with receipt of this registration form and that it is used for administrative purposes, such as insurance, team equipment, league fees, and other miscellaneous items needed to have a season - not to buy my child a uniform or a starting position. I understand that no refunds will be given after my child is drafted on a team. I also understand that the Vivian Athletic Association is subject to all mandates issued by state and local authorities, and the organization as a whole must abide by those mandates.

Vivian Athletic Association carries insurance on each child that is rostered in a timely manner. This is a secondary insurance policy which will only pay if I do not have personal insurance; however, I will be responsible for paying any and all deductibles, as well as the costs that are not covered by the Vivian Athletic Association's insurance policy. Any accident should have a report filled out within 72 hours. I fully understand and am in agreement with the above stated paragraphs. My child has permission to participate in the VAA Baseball Association. I further understand that the VAA is a volunteer non-profit organization, which will not be responsible for injuries that may occur to my child.
